Ask Your Primary Veterinarian for Medical Records

Medical Records Request

We have created a sample email you can send your primary veterinarian's office requesting the medical records we need for your appointment with the Behavior Service. Go to the sample and copy and paste it or use the information to call and request records for your pet to be sent to us.

Form FOR Referring Veterinarians

Referring Veterinarian's Form

This form is for veterinarians and their practices to submit medical records only in regard to consultations and appointments with the Behavior Service. This form should be filled out by a veterinarian (or employee of the veterinarian) who is or has managed the pet’s current health concerns.

This form should NOT be filled out by the pet owner.
